To slay the two demons Hiranyaksha and Hiranyakashipu, Lord Vishnu incarnated as Varaha and Narasimha. Before completely relinquishing the Varaha form used to kill Hiranyaksha, he incarnated as Narasimha in response to Prahlada's call. Hence, here we see the combined form of Varaha and Narasimha. After the destruction of the demon, Narasimha did not shed his fierce form, causing all the worlds to tremble. At that time, Brahma remembered the boon he had given to the sandalwood tree, which had the power to alleviate fierceness, heat, and distress. Upon Brahma's suggestion, Prahlada performed the sandalwood service, calming Narasimha's fierce form. That day was Akshaya Tritiya. The pacified Narasimha settled on Simhachalam at Prahlada's request.

The temple is notable for its Kalinga architecture and was built by the Eastern Ganga king Narasingha Deva I in the 13th century. The idol is covered with sandalwood paste throughout the year, except on Akshaya Tritiya, when the paste is removed to reveal the deity
