Abstract:
Introduction: High prevalence of health issues amongst rail workers, such as obesity (47%) and hypertension (45%) likely contribute to the 4.8% higher sickness absence rates compared to the national average. This may be linked to the nature of rail work, involving shift work, poor working environments, and a predominantly male-dominated culture resulting in reluctancy to seek health support. This highlights the need for deeper insights through qualitative analysis to understand health outcomes and create effective health interventions.
Methods: A mixed-method explanatory design was used. Quantitative data from health checks (n=800), informed qualitative exploration, including semi-structured individual interviews with rail workers (n=15) and qualitative observations of the environment and culture on 28 sites. An interview guide was designed based on the quantitative findings, and observation notes taken; data was analysed using thematic framework.
A cross-case joint display to compare qualitative data with quantitative scales was used to explore the influence of environmental/cultural factors on health outcomes, containing rows for each quantitative variable (e.g. smoking rates) and corresponding qualitative findings (e.g. working culture that allows smokers frequent breaks during shifts).
