The Annual Muthyalamma Devi car festival 2018, to be held in March-April every year, is a 400-year-old tradition that lakhs of devotees look forward to every year. The temple ceremonies, this festival goes on for a week, include pujas, homas, and processions. The area around the temple, on Muthyalamma Temple Street and Seppings Road Cross, wears a festive look throughout the period, with colorful stalls selling puffed rice, fried gram, and the usual ware instant markets offer.

Before the celebrations begin, the deity Muthyalamma is adorned with flowers on both the left and the right shoulder. The flowers on the right shoulder have a special significance — if they fall, it is considered to be a signal from the deity that preparations for the pujas and festivities can begin. The festivities begin with Vana Bhojana at Muneswara Temple in Pottery Town, where up to 2,000 people are fed. The following day, the palanquin bearing the deity is taken out in a procession on important roads such as Thimmaiah Road, Old Market Road, Seppings Road, and Jewellers Street.

Roughly a week later, on April 15, about 500 devotees with their lips and cheeks ritualistically impaled by steel and silver rods, come to the temple to offer prayers. Thousands of devotees then make an offering of beaten rice and jaggery shaped like lamps. The offering, called mavilakku, along with velari flowers, is made between 5 a.m. and 11 p.m..

Tradition has it that Muthyalamma is the grama devathe (village deity) of the Cantonment area, and Anyamma, is the grama devathe of the City area. Muthyalamma's history can be traced to the Great Plague that claimed hundreds of lives. The deity was installed to ward off the plague and diseases such as smallpox and cholera. Even today, devotees believe the goddess cures chronic diseases and grants any favour they seek, and devotees from among Hindus, Muslims, and Christians, seek her blessings. Devotees, who believe their wishes have been granted, make votive offerings of saris to the temple.

The saris in turn are donated to the poor by the temple authorities, and some of the ones donated are auctioned. The proceeds of the auction go towards temple funds. The tying of the talisman with the Muthyalamma inscription helps thousands of devotees ward off diseases. Incidentally, During the much-awaited cart festival, or rathotsava, the two temple chariots, one towering 70 feet, and the other 30 feet, are taken in a procession. The two chariots are meant for the two deities, Periya Muthyalamma and Chinna Muthyalamma.

Following the homa at 5 a.m., and the dollatsavam, the two chariots roll onto the streets to the accompaniment of nadaswara, a band set, thamate, and other traditional musical instruments. The chariots return to the temple at 9 p.m. when devotees make offerings such as salt, pepper, plantains, and coins to the chariot. Some of the devotees, who come from as far as Krishnagiri, and Tiruchi, besides villages around Bangalore, have their heads tonsured and ears pierced.

At 11.30 p.m., a chariot adorned with flowers is taken around in a procession again, this time accompanied by 50 other flower palanquins from temples of Anjaneya and Ganesha, in Nehrupuram, Vasanthnagar, and Shivajinagar. These palanquins move ahead of the majestic chariot. The car festival is a big day for the devotees who involve themselves in feeding those coming from far-off places. Some serve buttermilk and others prepare kosambari. Choultries in the neighborhood cook 15 to 20 bags of rice for mass feeding.
