Ostomy Appliance Change Demonstration with an Ostomy Nurse

Описание: Step 1: Gather all supplies needed for the appliance change

Step 2: Remove the old appliance gently and use the scent-proof bag to discard. Pull the skin down as the skin barrier is being pulled away to reduce the chance of trauma. Once removed, clean around and on the stoma with the wet and dry cloths

Step 3: Using the measuring guide, appropriately size the stoma. The goal is to be close around the stoma without forcing the guide around the stoma. Mark the appropriate size on the back of the skin barrier using the marker. Length and width measurements may need to be taken to accommodate an oval stoma.

Step 4: Cut along the marker line. When cutting, leave plastic backing in place. The curved edge of the scissors will go in the direction of the curve. Cut deep between the scissor blades to create a smooth edge on the skin barrier.

Step 5: Once cut, compare size to stoma and adjust if necessary. The skin barrier should not be forced over the stoma to fit. If excess skin is visible beneath the suture line, consider starting over with another skin barrier or if only a small amount is visible, fill in with adapt ring.

Step 6: Peel off plastic backing from the skin barrier. If using, stretch an adapt ring around the cut opening on the skin barrier. Prepare the skin for the skin barrier by drying it well. Use 3M Cavilon or stoma powder as needed or as ordered.

Step 7: Apply prepared skin barrier to skin applying pressure around the stoma and then securing the adhesive border edge by removing the paper tabs from the back of the skin barrier.

Step 8: Apply the pouch using the floating flange to the skin barrier. Close the pouch by creating a crease in pouch opening, then closing the Velcro using a "once forward, two back" technique.

Step 9: Hold hand over stoma to help warm the skin barrier to create better adhesion. When packing up supplies, make sure to save the plastic backing from the skin barrier for future reference.
