For GM it all began to go wrong along a stretch of highway in Georgia on March 10, 2010 when Brooke Melton, a nurse outside Atlanta, died in a crash. In this excerpt from "The Switch from Hell" we look at how the search for the truth of what happened in that crash led to the discovery of a deadly defect and a corporate coverup.
