This failure was is indifferent to other failures, it’s just in a bigger scale. There’s a problem with regulations of software updates deployment: just like in pharmaceutical industry there should be documented trials demanded for every CRC sum checked update.
The amount of trialed systems should be defined by amount of systems which are gonna be updated. Only after documented trials the software updates should be deployed, especially for any critical infrastructure or highly paid specialised software.
No. The test systems are not enough as many times they do not reflect the systems that software and software updates are deployed on to.
There should be regulations to make it happen.
