When children are diagnosed with scoliosis, the most common test done is the Adam's Forward Bending test. This is a screening test done to find indicators of scoliosis.
Early detection is important with scoliosis because it is progressive in nature, especially in children and adolescents.
The smaller curves are more flexible and easier to treat. Therefore, early detection from a conservative approach point is very important in preventing patients from ever dealing with severe scoliosis and preventing surgery from becoming the only recommended treatment.
However, an early diagnosis means nothing if the patient is not treated and left to watch and wait, which is the case for many patients referred to orthopedic surgeons. That is why early detection has somewhat gone away in routine checkups. If the curve is not big enough, it can't yet be treated from their point of view.
In the conservative approach, early detection is key. The younger the patient, the smaller the curve, the higher the odds of a positive outcome, slowing the curve's progression or preventing the patient from ever becoming surgical. Also, the smaller the curve is, the easier it will be to maintain it small with treatment.
The Adam's Forward Bending Test is a very easy scoliosis screening exam, which is why it is used so often.
The Adam's Forward Bending Test used to be done twice a year in schools. This was to try and catch a progression or change on an existing but barely noticeable curve.
The Adam's Forward Bending Test will not catch a small 5 or 10 degrees curve. Only after 25 degrees, and often thoracic scoliosis, will be diagnosed with this test.
This test involves having the patient bend forward to round the upper back to around 90 degrees, allowing the arms to dangle in a natural and relaxed position. The doctors will look for arching and spinal alignment when the patient is in this position. Doctors might also use a scoli-meter and place it over the spine to determine if there is a tilt.
This test will easily miss a lumbar scoliosis since it focuses more on detecting thoracic curves.
But it is known that finding scoliosis early and treating it right away will produce a much better outcome. That is why Dr. Tony Nalda does not recommend relying on this test, especially when it comes to children.
Between yearly pediatric visits, if a child has scoliosis and goes through a growth spur, that curve can grow exponentially and go undetected for an entire year, unless the growth is over 25 degrees, in which case it would be more obvious to the eye.
With teenagers is more of a challenge. As they begin to discover their body and start using bigger clothing and covering themselves up more, it gets harder to notice if a scoliosis shows or has progressed.
The only way to avoid surgical outcomes is to be vigilant with your family, especially children, focus on early detection in the years of more growth, and get regular screening done, not yearly ones.
