Home Health Devices Lowering Hospitalizations

Описание: One of the biggest challenges for hospitals is coming up with ways to keep people out. Readmission rates for the chronically ill and elderly are costly for healthcare and patients.

“We’re really working very closely with the teams in the hospital in trying to be sure that when people are discharged home that we get them as soon as possible and provide any of the products that we have that will keep them out of the hospital,” says Cathy Brady, who is a Telehealth manager with Lee Memorial Health System.

The answer may lie in take-home devices-that allow patients to live one their own with a little help.

“We provide the equipment into the patient’s home to teach them how to take their own vital signs,” says Brady.

With home monitoring equipment people take a variety of readings daily which are reviewed by health professionals. Tracking things like blood pressure, blood sugar and weight. It detects issues before they become emergencies; another device manages medications.

“I’ve had some people take as many as 20 medications,” says David Johnson.

Johnson distributes these devices for Lee Memorial Health System. Here and nationwide, medication mix-ups are a major issue.

“Where people forget. Not just forget to take their medications but sometimes forget that they actually took their medications,” says Johnson.

Providing medical oversight outside of brick and mortar hospitals helps patients stay independent.

“They want to live in their own home as long as possible,” says Brady.

And they can, by bringing a bit of health care home.
