Two really common terms associated with decreased sound tolerance are hyperacusis and misophonia. These terms actually look very similar in patients. Hyperacusis and misophonia are both aversions to sound. They can be related to volume or a specific type or a pattern of sound.

In hyperacusis patients, it really does not matter what the type of sound is. All sounds can become painful or uncomfortable. With misophonia, it's usually related to a pattern or a type of sounds like clicking mouth sounds, breathing, or coughing.

There is an overactivity of the nervous system and when the brain signals and flags a sound as being threatening or problematic, it will naturally activate our body’s protective mechanisms. Distress comes from the limbic system. Patients immediately experience a fight-or-flight response to things that other patients would find fairly normal.

For example, have you ever experienced where someone might sneak up behind you with an air horn? What happens to your body? Your heart rate elevates; you have a protective response. You might envision harming that person. This is the response of your body trying to protect you.

Now imagine if you went to work and it was "Bring Your Air Horn to Work Day." Everywhere you went, people were surprising you with that sound, and it produced this response in the body. This is what people with hyperacusis and misophonia experience all the time. They live in a world where it's almost as if everywhere you turn, there is an air horn, nails in a chalkboard, and some sound that is going to cause distress or pain to that person.
