Cystoscopy and Bladder Fulguration: Recurrent UTI with Dr. Angelish Kumar, Part 4

Описание: In this video Dr. Angelish Kumar, a New York-based urology specialist, discusses the following:

● Cystoscopies can be used to evaluate a patient’s bladder lining. This can be especially useful if a patient is experiencing irritative bladder symptoms that are not responding to antibiotics.
● Cystoscopies can reveal chronic inflammation (known as cystitis cystica) or smaller areas of ragged plaque-like texture. Samples can be taken for biopsy for a more accurate diagnosis.
● In her practice, Dr. Kumar is meticulous about ensuring a cystoscopy does not cause a patient pain, during or after the procedure. A numbing jelly is used on the urethra which is often dilated to accommodate the scope comfortably.
● Dr. Kumar estimates that in 60-70% of the cystoscopies she conducts, she is able to diagnose a condition which was previously unknown. Even when nothing is found, this helps Dr. Kumar determine the next treatment steps as it may point to other issues such as genitourinary menopause symptoms.
● Dr. Kumar explains that bladder trabeculation is simply a thickening of the muscle fibers in the bladder wall. Although it may increase the urgency to urinate, it is not a condition that typically requires treatment.
● In cases where a cystoscopy reveals inflamed bladder tissue, fulguration is an immediate treatment option. This involves cauterizing—or gently burning—the raised nodules of inflammation. A single treatment can significantly reduce UTIs in around 50-60% of patients. Some may require follow up treatments to achieve results.
● Fulguration is only suitable for small areas of inflammation, not excessive cystitis cystica. When treating small areas of the superficial, outermost layer of the bladder lining, the risk of scar tissue developing is very minimal.
● In cases of cystitis cystica, Dr. Kumar usually recommends a low-dose daily antibiotic like nitrofurantoin.

Dr. Kumar is a board certified Urologist who graduated with honors from The Tufts University School of Medicine and is a member of the prestigious Alpha Omega Alpha Honor Society. She completed her urology training at NYU Langone Medical Center in New York City. In 2017, she was the recipient of the gold star teaching award from the Albert Einstein College of Medicine. In 2020, she founded Women’s Urology New York, a unique practice focused on providing compassionate and comprehensive urologic care exclusively to women. In addition to being a diplomate of the American Board of Urology, Dr. Kumar is a member of the American Urologic Association, Society for Urodynamics and Female Urogenital Reconstruction, and International Urogynecologic Societies. She also serves on the Women’s Health Executive Committee for Springboard Enterprises.
