A guardianship form is used by the parents of current guardians of a child to list the caregiver of the child should something render the current parent unable to do so, temporarily or permanently.

Steps for completing your Guardianship Form on FormSwift:

1) Select your state

2) All guardianship forms will require some basic information that you must fill out. For more information regarding the components of your document, see “What is a guardianship form” on the landing page.

3) To complete the different fields of the document, simply add your information in as you are prompted on the left side of the document.

4) As with all FormSwift documents, you can electronically sign your form as well. Press the sign button next to your Guardianship form if you are looking at it in the My Documents tab of your account. If you are working on a PDF directly, you can sign your document by clicking the Signature button on the ribbon at the top of the page. Then choose whichever option suits your signature preference.

5) Once you are finished completing the document fields, click the green export button to the top right of your form.

6) Remember that this form generally needs to be notarized as well as signed and dated by the guardian and at least one witness.
