[Causes for Lean condition (Too much air in the fuel to air mixture)]

The prefect ratio for gasoline combustion engines is 14.7:1. I.e. for every one gram of fuel, 14.7 grams of air are required.

On the air side:
Dirty or failed /MAP sensor (ECU's calculations are wrong due to failed/dirty sensor)
O2 sensor issue (ECU's calculations are wrong due to failed/failing sensor)
Vacuum leaks (Allowing UNMETERED air into the system, thus, too much air)
Exhaust leaks (Allowing UNMETERED air into the system, throwing off the O2 sensors)

On the fuel side:
Dirty fuel injectors (ECU can't control the pulse width properly to adjust the ratio correctly)
Weak fuel pump (Fuel pressure is off, throwing off the ratio)
Clogged fuel lines. (Fuel pressure is off, throwing off the ratio)
