The common perception is that the nipple is removed and then put back during a breast reduction. In fact the nipple remains attached to a stalk of breast tissue and is repositioned to a higher position which means it is lifted.
Only in very large breast reductions or gigantic breast reductions is the nipple removed and replaced as a free nipple graft. However, this is rare and is only necessary in one or two percent of cases.
