Visceral leishmaniasis is a chronic parasitic condition which is caused by infection with Leishmania donovani. It usually occurs in the liver, spleen, lymph nodes, and bone marrow. This condition presents with its characteristic clinical features. The gold standard of treatment includes the demonstration of amastigotes. Whereas, serological tests like ELISA, and IFAT are used. Amongst the commonly used treatment strategies, Amp B and paromomycin are a few.
▬ 📃 Leishmania Donovani Disease
Visceral leishmaniasis is caused by the L. Donovani complex which includes L. Donovani and L. infantum. Apart from epidemiology, immunopathogenesis has been explained here. Diseased individuals cause Mɸ alternative activation/dysfunction alongside PMN dysfunction which leads to persistent infection and tissue damage.
▬ 📜 Visceral Leishmaniasis Diagnosis
Individuals with visceral leishmaniasis demonstrate hepatomegaly, lymphadenopathy, and hypoalbuminemia. The gold standard of diagnosis includes a demonstration of amastigotes in smears of tissue aspirates. Detection occurs via various serological tests which are ELISA and indirect immunofluorescent antibody tests (IFAT). Also, rapid immunochromatographic tests and PCR alongside differential diagnosis are described.
▬ 📃 Visceral Leishmaniasis Treatment
Amongst the various available treatment options, pentavalent antimicrobial compounds form a part of the treatment therapy. Moreover, amphotericin B along with liposomal AmB and paromomycin are also utilized to alleviate the condition. Besides, miltefosine is used. Moreover, multidrug therapy together with VL in immunocompromised hosts is explicated.
