1. Beighton Score
Purpose: Simple bedside screening tool for generalized joint hypermobility.
Method: 9‑point scoring system based on 5 maneuvers:
Passive dorsiflexion of little finger greater than 90° (1 point each hand).
Passive apposition of thumb to forearm (1 point each hand).
Hyperextension of elbow greater than 10° (1 point each arm).
Hyperextension of knee Greater than 10° (1 point each leg).
Forward flexion of trunk with palms flat on floor (1 point).
Interpretation:
Adults: ≥5/9 indicates hypermobility.
Children/adolescents: ≥6/9.
Strengths: Quick, widely used, reproducible.
Limitations: Focuses only on a few joints, doesn’t assess symptoms or systemic features.
2. Brighton Criteria
Purpose: Broader diagnostic framework for Joint Hypermobility Syndrome (JHS), now often considered part of the hypermobile Ehlers–Danlos spectrum.
Components:
Major criteria:
Beighton score ≥4 (current or historical).
Arthralgia for greater than 3 months in ≥4 joints.
Minor criteria:
Beighton score 1–3.
Arthralgia in 1–3 joints or back pain greater than 3 months.
Dislocations/subluxations.
Soft tissue rheumatism (e.g., bursitis, tendonitis).
Marfanoid habitus.
Skin striae, hyperextensibility, thin skin.
Eye signs (drooping eyelids, myopia).
Varicose veins, hernia, uterine/rectal prolapse.
Diagnosis:
2 major criteria, OR
1 major + 2 minor, OR
4 minor.
Strengths: Considers systemic features and symptoms.
Limitations: More complex, less practical for quick screening.
3. Hospital del Mar Criteria
Purpose: Alternative scoring system developed in Spain to assess hypermobility more comprehensively.
Method: Evaluates 11 maneuvers across multiple joints (fingers, wrists, elbows, shoulders, hips, knees, ankles, spine).
Scoring: Each maneuver scored; higher total indicates greater hypermobility.
Strengths: Broader joint coverage than Beighton.
