What is HLA Typing?
HLA typing stands for Human Leukocyte Antigen typing. It’s a genetic test commonly used by transplant physicians to determine compatibility between a donor and a recipient. The goal in transplant medicine is to find the closest HLA match so the recipient’s immune system doesn’t reject the transplanted organ. The higher the match between HLA alleles, the better the chances of acceptance.
Why Is It Done in Fertility Treatments?
When couples face repeated implantation failure or recurrent pregnancy loss, fertility experts suspect there might be an immunological cause. One of those causes is excessive HLA similarity between partners. In fertility, too much similarity between a male and female partner's HLA alleles can lead to the maternal immune system failing to recognize the embryo as a semi-foreign body. As a result, it doesn't mount the protective immune response needed to support the pregnancy.
What to Look for in a Report!
In the sample report, you’ll see HLA alleles listed for both partners. Let's say each partner has 6 alleles across different loci — that's a total of 12. A report might say 3 out of 12 alleles match. That means a 25% match.
Key Risk Factors Based on Report
If both partners carry HLA-C2 type alleles (C2/C2 combination), it increases pregnancy loss risk. Similarly, if the report shows over 50% HLA allele matching, the immune system may fail to respond appropriately to the embryo, increasing the chance of rejection.
Does a Low-Match Report Guarantee a Healthy Pregnancy?
No. This test does not provide a guarantee. A low match only reduces one possible risk factor. For this report to be relevant, the couple must fulfill three important conditions:
They must have a normal karyotype (genetic testing must be clear for both).
The embryos transferred should be chromosomally normal (euploid).
There should be no other anatomical or hormonal abnormalities.
Without fulfilling these conditions, HLA typing won’t help in drawing meaningful conclusions.
Is HLA Typing Always Necessary?
No. It is only advised when:
There’s repeated unexplained IVF failure with good-quality embryos.
