Lead poisoning has profound effects on the blood and bone marrow, disrupting normal hematologic function and impairing red blood cell production. These effects are especially dangerous in children, whose developing systems are more vulnerable to toxic insults.
🩸 Effects on Blood
Lead interferes with several key enzymes in the heme synthesis pathway, leading to:
1. Anemia
Microcytic hypochromic anemia is common.
Caused by impaired hemoglobin production and shortened red blood cell lifespan.
2. Basophilic Stippling
Seen on peripheral blood smear.
Represents aggregated ribosomal RNA in red blood cells due to disrupted maturation.
3. Inhibition of Enzymes
δ-Aminolevulinic acid dehydratase (ALAD) and ferrochelatase are inhibited.
This leads to accumulation of aminolevulinic acid (ALA) and protoporphyrin, which are toxic to cells.
4. Elevated Free Erythrocyte Protoporphyrin (FEP)
A useful biomarker for lead exposure.
Indicates impaired incorporation of iron into heme.
🧬 Effects on Bone Marrow
Lead affects the bone marrow—the site of blood cell production—in several ways:
1. Suppression of Erythropoiesis
Lead disrupts the maturation of red blood cell precursors.
Results in ineffective erythropoiesis and anemia.
2. Marrow Toxicity
Chronic exposure may cause marrow fibrosis or hypoplasia.
Can impair production of other blood cell lines (white cells and platelets).
3. Storage in Bone
Lead is stored long-term in bones and teeth.
During periods of increased bone turnover (e.g., pregnancy, lactation), lead can be released into the bloodstream, re-exposing the body.
⚠️ Clinical Implications
Children are especially vulnerable due to higher absorption rates and developing hematopoietic systems.
Even low levels of lead can cause neurodevelopmental delays and behavioral problems, in addition to hematologic effects.
Chelation therapy may be used in severe cases to reduce blood lead levels, but it does not reverse bone marrow damage.
