A patient with prior heart transplant presented with recurrent in-stent restenosis of the right coronary artery (RCA) due to a balloon undilatable lesion. Engagement of the RCA was challenging due to right-sided aortic arch, but was eventually successful after using a diagnostic catheter for engagement, inserting a Grand Slam guidewire to the distal RCA and advancing a JR4 guide over the Grand Slam guidewire.
Orbital atherectomy and intravascular lithotripsy were performed with some improvement in expanding the RCA lesion. A year later the patient presented with RCA stent thrombosis. Despite multiple balloon inflations, thrombectomy and administration of vasodilators and a glycoprotein IIb/IIIa inhibitor antegrade flow could not be restored to the RCA.
