On April 10, 2010, a Tupolev Tu-154 aircraft operating as Polish Air Force Flight 101 crashed in foggy weather while attempting to land at an airport near Smolensk, Russia. All 96 occupants were killed, including the then-president of Poland and several other notable people. Many relatives of victims of the Katyn Massacre were also onboard. The aircraft was traveling from Warsaw to Smolensk to visit the site of the Katyn Massacre (not far from Smolensk) for the massacre’s 70th anniversary. The probable cause was cited as the cockpit crew’s failure to respond to a series of warnings issued to the crash prior to the impact (these warnings can be heard in the video) which warned the crew of their dangerous altitude and instructed them to pull up and begin a climb. This crash is an example of CFIT, or ‘controlled flight into terrain.’ The plane was fit for service and the crew was in full control, and despite this, they continued their descent until the aircraft struck trees and immediately thereafter impacted the ground. The bad visibility and foggy weather during the crash were also cited as contributing factors.
