The 18k yellow gold case of the Patek 5110J possesses a very smooth design with gently-sloping curves throughout. All the case elements, from the lugs to the 'crown guards' to the bezel, appear to meld together into a single piece. Likewise, the line of the curved bezel is followed by a domed, scratch-resistant sapphire crystal.

The dial is presented in a silver-tone with a guilloche center dial that is adorned with applied, 18K yellow gold hour indices. The hands at center are also in yellow gold with a high-polish finish and in a very sharp conformation.

There is also a silver/black dual-tone ring surrounding the center dial that serves as an AM/PM indicator. Surrounding the latter is yet another silver ring upon which the 24 time-zone reference cities are imprinted. It is this outermost city ring and the dual-tone ring which rotate to properly indicate the time in different cities throughout the world.
