When a baby is born prematurely, the brain is still developing and extremely vulnerable. In the NICU, careful oxygen management is critical — because both too little oxygen and too much oxygen can cause serious brain injury.

Periventricular leukomalacia (PVL) is a type of white matter brain injury that can occur in premature infants. White matter acts as the brain’s wiring system, helping control movement and coordination. When white matter tissue dies, it can lead to cerebral palsy and other long-term movement disorders.

In the NICU, premature babies may receive oxygen support through:

• Mechanical ventilation (intubation)
• CPAP
• Nasal cannula oxygen

Regardless of the method used, oxygen levels must be carefully monitored at all times. Oxygen saturation targets, blood gases, and carbon dioxide levels must be maintained within safe ranges. Fluctuations in oxygen or improper ventilation management can increase the risk of brain injury.

Proper NICU care requires constant monitoring, rapid response to desaturations, and precise adjustment of ventilator settings. The premature brain cannot tolerate instability in oxygen delivery.

If your child was born prematurely and later diagnosed with PVL or cerebral palsy, it is important to review NICU ventilation records, oxygen saturation trends, and blood gas levels carefully.
