There is high prevalence of unsuspected intrauterine lesions in females with unexplained infertility. So this could be in the form of small adhesions polyps or there could be even partial or complete septum and sometimes these intrauterine lesions are not picked up by the normal transvaginal ultrasonography. But these lesions significantly reduces the pregnancy outcome you can have difficulty in conceiving spontaneously and even with these intrauterine lesions success rate with IVF becomes less. So here comes the role of hysteroscopy is a very tiny instrument with almost diameter of around 2 to 3 mm with camera and light attached to it. So under anesthesia we introduced this tiny hysteroscope through vagina into the cervix and into the uterus and with the help of light and camera we can very clearly see inside the uterine cavity. How is the cavity, we could also visualize the bilateral tubal openings. If there are any adhesions present if there is any polyps or any septum in the same sitting we can correct it. So nowadays very tiny hysteroscopic instruments are available in the form of scissors. So through the help of these cesarean we can correct these intrauterine lesions in the same sitting we can remove the polyp, we can incise the septum also we can do the adhesiolysis. After hysteroscopy and after the correction of these lesions patient can conceive spontaneously also and even if we do hysteroscopy pre IVF before the embryo transfer. Now lot of evidence says that it helps in you know increasing the implantation rate and the IVF success rate and life birth rate increases.
