These are generally available at Home Depot and Lowes, and while sometimes tricky to install, are among the best of their type. One thing to understand though is your HDMI and audio cables need to go in at the time of installation, not afterward. This is to comply with building codes and rules which prohibit extension cords inside of walls.
Installing these can be a bit of a hassle but is usually done as an ad-on to a TV installation. Figure a skilled installer will take just over an hour, while standard electrical labor units on this may count this as two outlet devices (.75hrs X2, so 1.5 man hours billable labor). That's because it requires cutting two holes and wiring two items, one per hole, plus fishing 1-4 other cables through the wall.
TV installers will generally discount this when done in conjunction with another service call, such as a TV wall mounting. When performed as a standalone service, the minimum charge is usually two hours, maximum is three, including setup and cleanup time.
