The clear is done using an Aughild setup. When used alongside the Sankis axe, you end up being a little tougher than when using the standard CoE setup alongside the Relentless passive (if you use neither the Aughild setup nor Relentless with CoE, you will generally die).
The main issue with this setup is that because Aughild shoulder displaces Fury of the Ancients, you don't passively generate Fury. It's fine when you're fighting a single target, like a boss, and Gavel generates Fury for you, but when fighting in density, Gavel doesn't activate (you need to hit 3 or fewer enemies with your HotA) and you'll run out of Fury almost instantly. So, to make the build playable, you must take the Weapons Master passive, and make sure you are holding Remorseless in your main hand (the passive bases its effect only on your main hand weapon). This gives you Fury for every enemy hit. You just need to make sure you are fighting in decent density or your Fury will start to run out.
It is MUCH harder getting a 150 clear with IK HotA than it is with LoD HotA. That said, I think it is my preferred version of HotA, since I really enjoy the way Pain Enhancer enables big pops of damage in good density.
