At the VICS/ACSC cancer survivorship Community of Practice event on 26 February 2026, 40+ expert stakeholders gathered at the Peter MacCallum Cancer Centre and another 40+ online discussed preparing cancer patients for the post-treatment ‘survivorship’ phase of care.
This recording collects their findings from part 2 of the discussion, focusing on the prompts:
• How do you prepare patients for what to expect after their initial treatment ends?
• Please describe any enablers or barriers in current practice that impact how well (or not) you are able to support patients transitioning to survivorship and follow-up care.
• After this discussion, and thinking about your role, what is something you could do to improve support to patients transitioning into the follow-up stage of care?
