Ketamine, also called racemic ketamine, contains two mirror-image forms of the same molecule: S-ketamine (esketamine) and R-ketamine (arketamine). Esketamine (Spravato) is one of these two forms of the molecule. Esketamine is often delivered intranasally and racemic ketamine is often delivered intravenously (IV). The “mirror-image” difference and the way they're typically administered make a difference.

The racemic mixture of ketamine looks to be more effective than either esketamine or arketamine alone, and IV ketamine infusions are more available, and can be more carefully controlled, than other methods of delivery including intranasal administration.

While both may be effective, compared to intranasal esketamine, or Spravato, the information we have suggests that IV infusions of ketamine are more effective.
