Dr. Centeno explains that clicking and popping sounds in the upper neck—termed crepitus—are often signs of craniocervical instability (CCI), where excessive motion in the neck joints leads to these noises. While sometimes caused by harmless factors like joint movement or air bubbles, in CCI, crepitus typically indicates that joints such as C0-C1, C1-C2, or C2-C3 are unstable. The exact location of the crepitus can help pinpoint which cervical level is involved: C1-C2 issues may worsen with head rotation, while C2-C3 instability may be linked to dizziness or imbalance. Recognizing and localizing crepitus can aid in achieving a more accurate diagnosis for patients with CCI.
