Anemia Classification
Anemia is a hematological condition characterized by a decrease in the number of red blood cells (RBCs) or a reduced concentration of hemoglobin in the blood, leading to impaired oxygen transport to body tissues. Proper classification of anemia is essential for accurate diagnosis, understanding the underlying pathology, and planning effective treatment. Anemia can be classified on the basis of morphology, etiology (cause), and pathophysiology.
🔬 1. Morphological Classification (Based on RBC Size & Hemoglobin Content)
This classification is based on RBC indices such as MCV (Mean Corpuscular Volume) and MCHC (Mean Corpuscular Hemoglobin Concentration).
• Microcytic Hypochromic Anemia
RBCs are smaller than normal and contain less hemoglobin. Commonly seen in iron deficiency anemia, thalassemia, and sideroblastic anemia.
• Normocytic Normochromic Anemia
RBCs are normal in size and hemoglobin content but reduced in number. Seen in acute blood loss, hemolytic anemia, and anemia of chronic disease.
• Macrocytic Anemia
RBCs are larger than normal. It is further divided into:
– Megaloblastic anemia (due to vitamin B₁₂ or folic acid deficiency)
– Non-megaloblastic anemia (seen in liver disease, alcoholism, hypothyroidism)
🧬 2. Etiological Classification (Based on Cause)
• Blood Loss Anemia – Due to acute or chronic hemorrhage (trauma, GI bleeding, menstruation).
• Nutritional Anemia – Caused by deficiency of iron, vitamin B₁₂, folic acid, or proteins.
• Hemolytic Anemia – Premature destruction of RBCs (sickle cell anemia, malaria, autoimmune hemolysis).
• Aplastic Anemia – Bone marrow failure leading to reduced RBC production.
• Anemia of Chronic Disease – Associated with chronic infections, inflammatory disorders, or malignancies.
⚙️ 3. Pathophysiological Classification (Based on Mechanism)
• Decreased RBC Production – Due to bone marrow disorders or nutritional deficiencies.
• Increased RBC Destruction – Seen in hemolytic anemias.
• Blood Loss – Due to internal or external bleeding.
