A woman who is four months pregnant may not appear to be "showing" if she has a tilted, or retroverted, uterus. Normally, the uterus tilts slightly forward, but in about 20-30% of women, it tilts backward toward the spine. In early pregnancy, this can cause the growing uterus to remain closer to the back of the pelvis, which can conceal a visible baby bump, even at four months. This backward position may keep the abdomen flatter, delaying the appearance of pregnancy.
As the pregnancy progresses, the uterus usually shifts forward, and a visible bump becomes more noticeable. A tilted uterus is generally harmless and doesn’t affect the health of the pregnancy, but it can lead to a delayed “showing” that surprises both the woman and those around her. This normal variation in anatomy is one of several reasons why some women may not look visibly pregnant in the early to mid-stages of pregnancy.
