In the Pennsylvania blast furnace district in the early 1900s, a maintenance worker entered a Cowper stove on a scheduled inspection. The purge had run its prescribed interval. The procedure said the vessel was safe.
He didn't come back up. A second man went in to check. The accident report said overcame by gas. The procedure was not changed.
This is the story of the hot blast stoves — 100-foot pressure vessels burning 25% carbon monoxide, and the maintenance workers who entered them with nothing but a schedule and their own senses.
The interval was a guess dressed as a standard. The gas didn't distribute the way the guess assumed.
