What Is Care Management?
Care management is a patient-centered, team-based approach that helps individuals—especially those with chronic conditions—stay healthy, avoid unnecessary hospital visits, and get the support they need between appointments.
Nurse Care Managers play a key role in:
Reviewing medical records
Following up with patients
Monitoring for red flags
Educating on lifestyle and medications
Coordinating with doctors, pharmacies, specialists, and caregivers
Making sure nothing falls through the cracks
Real-World Example:
Just recently, I spoke with a patient who was experiencing sudden lip swelling. Immediately, I recognized the signs of angioedema, a possible reaction to certain blood pressure medications. I advised her to stop the medication and contact her doctor right away. The next day, I followed up to ensure she received care and a new prescription.
That’s what Nurse Care Managers do—we bridge the gap between office visits and everyday life. We catch what others miss. We follow through when the system stops short. And we make sure patients feel seen, heard, and safe.
Do You Need Certification?
No. If you’re a licensed RN or LPN, you’re already qualified to be a Nurse Care Manager. While certifications exist, they are optional—not required to start your own care management business or deliver services like Chronic Care Management (CCM) or Remote Patient Monitoring (RPM).
What you do need is a system, a plan, and the right support—and that’s what this channel is here for.
