The CGHS card charges depend on the basic pay (as per 7th CPC) of the Central Government Employee. The monthly subscription rates for serving employees are as follows:

CGHS Contribution Rates (As per 7th CPC)

For Pensioners (Lifetime Contribution)

Pensioners can choose either a lifetime contribution (one-time payment) or a monthly deduction.

Additional Charges for Plastic CGHS Card

₹50 per card (to be deposited at the CGHS Wellness Centre).
1. Online Application Process (Preferred Method)

You can apply through the CGHS Web Portal:

Steps:

1. Visit the CGHS website and go to the "Beneficiary" section.


2. Click on ‘Apply for CGHS Card’.


3. Register/Login using your credentials.


4. Fill the online form with details:

Employee ID

Date of birth

Department details

Residential address

Family members' details (if applicable)



5. Upload necessary documents (scanned copies in PDF format).


6. Submit the form and note the reference number for tracking.


7. Print the online application and submit it to your department for further processing.

2. Offline Application Process

You can also apply manually by submitting a physical application.

Steps:

1. Download the CGHS Card Application Form from the CGHS website or collect it from the nearest CGHS Wellness Centre.


2. Fill the form with the required details.


3. Attach required documents (see below).


4. Submit the completed form to your department’s CGHS office or administrative officer.


5. The department will verify the details and forward it to the CGHS office.


6. Once processed, you will receive your CGHS card.
