n January 8, 2005, the USS San Francisco (SSN-711), a Los Angeles-class nuclear attack submarine, collided with an uncharted undersea mountain while traveling at high speed approximately 360 nautical miles southeast of Guam. The submarine was submerged at a depth of around 500 feet when it struck the seamount head-on.
The impact was catastrophic—the sonar dome was completely destroyed, forward ballast tanks crushed like an accordion, and the bow section devastated. Despite the extreme damage, the pressure hull and nuclear reactor remained intact, preventing what could have been a far worse disaster.
Tragically, Machinist's Mate 2nd Class Joseph Ashley lost his life in the collision, and nearly 100 crew members sustained injuries ranging from minor to severe. The submarine managed to surface using emergency procedures and returned to Guam under its own power—a testament to both the vessel's engineering and the crew's skill under extreme conditions.
This incident raised serious questions about navigation procedures, chart accuracy, and the risks faced by submarines operating in poorly mapped ocean regions. The collision resulted in a major investigation and changes to submarine navigation protocols.
