UWorld High-Yield: Pneumonia & CURB-65 | When Lungs Start Making Rice Krispies

Crackles, fever, yellow sputum, leukocytosis, and that signature mild hyponatremia.
Chest X-ray shows consolidation because air left the group chat.

CURB-65 tells you the admission plan:

C – Confusion
U – Urea above 20
R – Respiratory Rate 30 or more
B – Blood Pressure low (systolic around 90 or diastolic around 60)
65 – Age 65 or older

Score 0–1: Outpatient.
Doxycycline or a macrolide.
Hydrate and pretend everything is okay.

Score 2: Admit to hospital.
Beta-lactam plus macrolide or a respiratory fluoroquinolone.
Avoid fluoroquinolones in older patients because they love causing C diff, tendon rupture, and aortic drama.

Score 3 or more: ICU time.
Beta-lactam plus macrolide.
Or beta-lactam plus doxycycline if macrolides are cancelled today.
