What are the physical signs of scoliosis? Scoliosis is characterized by an unnatural sideways spinal curvature. Additionally, the spine rotates, making scoliosis a three-dimensional condition. The rotation typically occurs into the concavity of the curve. For a diagnosis of scoliosis, the curve must measure at least 10 degrees. This measurement is taken using the Cobb angle during an X-ray, which indicates the severity of the scoliosis.
The larger the Cobb angle, the more pronounced the curvature and the more severe the condition. Physical manifestations of scoliosis become more evident as the severity increases.
Scoliosis severity is categorized as follows:
Mild: Cobb angle between 10 and 25 degrees.
Moderate: Typically between 25 and 40 degrees.
Severe: Anything over 40 degrees.
Very severe: 80 degrees or more.
Scoliosis can progress, especially during growth spurts. While the condition may progress slowly during adult years, it can become rapid later in life. Physical changes from scoliosis can be significant during growth stages, but they can also become more pronounced in the later stages of life. Mild scoliosis can often go unnoticed, while moderate and severe forms are more evident.
The type of scoliosis also influences its appearance. S-shaped curves often appear more balanced, while C-shaped curves are more conspicuous. It's essential to understand that the progression isn't linear; for instance, a curve that increases from 10 degrees to 20 will appear twice as severe. From 20 to 30, the visual severity doubles again.
Common physical signs of scoliosis include posture deviations, uneven shoulders and shoulder blades, rib arching (especially evident during the forward bending test), uneven hips, uneven leg and arm spacing, a noticeable lean to one side, improperly fitting clothes, and changes in gait, balance, and coordination. Notably, any posture asymmetry in a child is abnormal and should be evaluated.
A frequent misconception is that scoliosis isn't severe if there's no pain. Children and adolescents typically don't feel pain because the spine isn't compressing tissues and nerves. While they might experience mild pain at the curve site, it's usually not distinct from other common aches. In adults, as the curve progresses, there can be nerve and tissue compression, leading to pain. This can manifest as back pain, mid-back pain, or pain radiating to the hands and feet. Some adults only discover they have scoliosis after seeking medical attention for pain.
