Patient Priorities Care is an approach to health care that aligns care among clinicians, patients and caregivers with what matters most to patient. It recognizes that patients are the experts in what they want to achieve from their healthcare, while clinicians are the experts in how to get them there. It is especially suited for meeting the health care needs of adults with multiple chronic conditions.
This video introduces the Patient Priorities Care approach and how it is used for clinical decision making for older adults.
