Is dapoxetine available in the US? How safe is the alternative?

This gets discussed on reddit and various forums too. Now I'm not a pharmacist or a doctor so I can only express my personal opinion, but as a therapist who works with the issue of premature ejaculation, I can hopefully clear up a couple of things.

In a nutshell, dapoxetine is a medication that aims to delay ejaculation. It's one of the SSRI antidepressants, but has a key feature: it can be taken on-demand rather than every day. It absorbs quickly into the body and doesn't hang around too long, so guys can get the delay effect when they need it.

Dapoxetine has been around for 20 years or so and is approved and marketed in over 50 countries. It's usually known under the brand Priligy - especially across Europe and the UK.

In other parts of the world, dapoxetine is branded as Daplong, Everlong, Xydap and Joypox.

Here's the thing: dapoxetine isn't approved in North America. The US Food and Drug Administration turned down the application in 2005. Pharmaceutical companies will no doubt continue to develop and reapply, and the FDA could pass approval someday, but right now - it's a no.

As for why dapoxetine isn't FDA approved, we don't know - that's not publicly available information. But pharmaceutical industry observers say that safety was a likely issue.

All SSRI antidepressant meds have potential side-effects, but it's possible that the on-demand, irregular use of dapoxetine doesn't allow for the 'settling in phase' that often takes a couple of weeks with a daily SSRI pill. That's speculation on my part, but maybe.

So some guys in the US and other non-approval places are turning to online pharmacies. Some websites are selling combination drugs of dapoxetine mixed with tadalafil, or bottles of liquid dapoxetine.

My advice: don't go down that route. You don't know what you're going to get, if you get anything at all. Some of these sites take payment only in crypto, or say 'not for human consumption, lab research only'.

If you're considering medication for premature ejaculation, or erectile difficulties, or both, go and talk to your doctor about trying an SSRI prescription or other options. Get a physical checkup too, that's a completely sensible way to go.

And if dapoxetine isn't a legitimate option in your country, are you really missing out? From my understanding of the research, and anecdotally from men who've tried it, it can slow things down and help men feel a bit more confident in bed. The effect is real.

But not for every guy and the improvement isn't massive - maybe from one to two minutes, say. And then there are those potential side-effects, and it's not a lasting treatment of course. When you don't take dapoxetine on the day, say if sex happens spontaneously, you might feel back to square one and all the pressure that brings.
