Treating valvular heart disease with transcatheter procedures has been there in the form of mitral valvotomy for a long time, but ability to change the aortic valve by TAVI or TAVI-RADICIT is called has been very popular for the last one decade or so. We have been pioneering these techniques, both TAVI as well as mitral valve procedures, TAVR. TAVI now has become such a common procedure, especially in those high-risk patients and the surgical turn downs, and we are able to deliver 99 % percent plus success in these patients. Most, many of these patients are very elderly, very frail and we have to take tremendous care in doing the vascular access and these procedures. We are very happy to tell that our results have been not only successful in the immediate term, but in the long term as well. The results were gratifying. Seeing very elderly 80 year olds coming up with tremendous problems and then seeing them walking around back home is a real gratifying experience. TAVI in our own experience has evolved over the last one decade. Now we are able to do most of the procedures from the groin with simplified procedures with simpler steps and with simpler finer catheters and valves. All sorts of aortic valve diseases can be addressed. All varieties of valvular stenosis in the aortic valve, whether by birth congenital or sclerosis degenerative diseases in the elderly can be tackled with appropriate balloon expandable or self expandable valves and we have tremendous experience with all sorts of valves. In those patients of TAVI where some leaks have been left aside in some people, they do come across from all over the world to our center. We do treat them with paraviral leak closure using vascular plugs. Again, by a trans-catalytic technique without taking the course to open surgery. The intermediate and the long-term outcomes of TAVI or TMVR have been comparable to open heart surgery. The valves are the tissue valves like in open heart surgery. And these patients do very well in the intermediate or long term as well. We do have results now going up to eight to 10 years with sustained benefit for these patients.
