A stroke occurs when the blood supply to part of your brain is interrupted or reduced, preventing brain tissue from getting oxygen and nutrients. Brain cells begin to die in minutes.A stroke is a medical emergency, and prompt treatment is crucial. Early action can reduce brain damage and other complications.
Ischemic stroke is characterized by the sudden loss of blood circulation to an area of the brain, resulting in a corresponding loss of #neurologic function. Acute ischemic stroke is caused by thrombotic or embolic occlusion of a cerebral artery and is more common than #haemorrhagic stroke.
Stroke is a major cause of mortality and morbidity, and thrombolysis has served as a catalyst for major changes in the management of acute ischaemic stroke.More than two-thirds of the global burden of stroke occurs in developing countries, where the average age of patients is 15 years younger than in developed countries
#Thrombolytic therapy is of proven and substantial benefit for patients with acute cerebral ischemia.More than 80% of the strokes are acute ischaemic strokes (AIS) requiring quick restoration of the blood flow to brain areas blocked by a vessel occlusion. Although thrombolytic agents were identified in the 1930s, their development in AIS was hampered by the lack of diagnostic opportunities to exclude #intracerebralhaemorrhage.
TYPES
An interruption of blood supply to the brain causes all ischemic strokes. However, ischemic strokes can start in different areas of the body and may occur due to various types of blockage:
Embolic stroke: This occurs when a blood clot, piece of plaque, or another object forms in a different part of the body and travels to the brain's blood vessels.
Thrombotic stroke: This type of stroke occurs when a thrombus or clot forms inside a blood vessel in the brain.
CAUSES
An ischemic stroke occurs due to atherosclerosis, which causes fatty deposits and a build up of cholesterol plaque in the blood vessels.When too much plaque collects in one spot, it can block the blood flow to vital organs.A blood clot may stick to a plaque buildup while passing through the blood vessels, which also creates a blockage.
The neck contains carotid arteries, which are blood vessels that supply blood to the brain. If plaque blocks a carotid artery (carotid artery disease), a stroke might occur.
Atherosclerosis is largely symptomless. Therefore, many people do not know they have atherosclerosis until they have a stroke or experience the other effects of blocked arteries near different organs, such as a heart attack.
THROMBOLYTIC THERAPY
Thrombolytic therapy is the administration of drugs called lytics or “clot busters” to dissolve blood clots that have acutely (suddenly) blocked your major arteries or veins and pose potentially serious or life-threatening implications. To be effective, the therapy needs to be initiated as soon as possible, before permanent damage has occurred.
The length of a treatment session varies depending on the underlying cause.
There are two ways clot-busting agents (lytics) can be given: through a peripheral IV (systemic thrombolysis) or through a catheter (thin tube) that has been navigated to the site of the clot.
SYSTEMIC THROMBOLYSIS is used for heart attack, stroke and pulmonary embolism.
• The “clot-busting” drug will be delivered through a peripheral intravenous (IV) line, usually through a visible vein in your arm. 
• Performed at your bedside in an intensive care unit while your heart and lung functions are monitored.
• The drug circulates within the blood stream until it reaches the clot. This may lessen the drug’s effect because of dilution, or higher doses may be needed increasing the chance of bleeding.
A stroke can be frightening and may occur in a person who seems to be in good health. Knowing the warning signs and seeking emergency medical care are the most important ways to improve a person's outlook after a stroke.If a stroke is happening to a friend or family member, people should not drive them to a hospital. Instead, they should call an ambulance, so that paramedics can provide medical care as quickly as possible.An ambulance can also take the person to the hospital that can provide the best stroke care, which is not always the closest facility.A rapid response significantly improves a person's chances of survival after a stroke.
