Commands:
Custom tag item
/give @p minecraft:iron_sword{Custom:11}
or
/give @p minecraft:iron_sword{Custom:11,display:{Name:'{"text":"Poison Sword","color":"green","italic":false}'}}
Scoreboard
/scoreboard objectives add hit minecraft.custom:minecraft.damage_dealt
Effect
/execute at @a[nbt={SelectedItem:{tag:{Custom:11}}},scores={hit=1..}] run effect give @e[distance=..7,nbt={HurtTime:10s}] minecraft:poison 10 10
You can change any effect you want
e.g.
/execute at @a[nbt={SelectedItem:{tag:{Custom:11}}},scores={hit=1..}] run effect give @e[distance=..7,nbt={HurtTime:10s}] minecraft:wither 10 10
Scoreboard reset
/scoreboard players reset @a hit
Holding effect
/effect give @a[nbt={SelectedItem:{tag:{Custom:11}}}] minecraft:speed 2 2 true
Holding particle
/execute at @a[nbt={SelectedItem:{tag:{Custom:11}}}] run particle minecraft:enchant ~ ~1 ~ 0.5 0.5 0.5 0.1 1 normal @a
