Lethargy:
When spoken to in a louder voice, the patient appears drowsy but opens their eyes, looks at you, responds to questions, and then falls back asleep. This indicates a decreased level of wakefulness and arousal.
Obtundation:
When tactile stimuli, such as gentle shaking, is applied, the patient opens their eyes, looks at you, but responds slowly and appears somewhat confused. Their alertness and engagement with the environment are notably decreased.
Stupor:
The patient arouses from sleep only in response to painful stimuli. Verbal responses are slow or absent. When the stimulus stops, the patient lapses back into unresponsiveness, indicating minimal awareness of self or surroundings.
Coma:
Despite repeated applications of painful stimuli, the patient remains unarousable with eyes closed. There is no evident response to internal needs or external stimuli, suggesting a complete lack of wakefulness or awareness.
