The major reason I like to do a CADCAM guide is to perform flapless surgery. However, case selection for flapless surgery is absolutely critical. Placing implants accurately with the flap raised is not something I find too difficult, so I don't typically make a guide in situations when I'm raising a flap, which is the majority of cases.
I find that when there's a lot of bone and a lot of keratinized tissue, CADCAM guides with flapless surgery makes things an absolute cakewalk, especially for the patient, with minimal post surgical complaints and no irritating sutures.
