Fall protection anchor failing on site

When an anchor is mounted in concrete, it needs to be proof loaded to 50% of it's load rating. Anchors are load rated to 15Kn for one user for fall restraint (work positioning) or fall arrest, and normally the same if you're abseiling.
Last Thursday one of our Height Safety Inspectors was doing a 6 monthly check on anchors at this site, and it pulled straight out without the dial on the load tester even registering.
If a worker had been relying on that anchor, they would have fallen off the edge of the building and died.
