In this video, Dr Kenneth Cutbush, Orthopaedic Shoulder Surgeon in Brisbane, presents a rare and interesting case of posterior shoulder instability caused by a posterior labral tear. While 80–90% of shoulder instability cases are anterior, posterior instability is far less common and often associated with significant bone loss and traumatic shoulder events.
Dr Cutbush walks through the patient’s MRI arthrogram, showing the dye-filled joint, rotator cuff anatomy, and the distinct posterior labral tear visible as a white line separating the labrum from the glenoid rim. He then reviews a 3D CT reconstruction, revealing substantial posterior glenoid bone loss—approximately 25%—as well as a reverse Hill-Sachs lesion, indicating the humeral head impacted the posterior glenoid during the dislocation.
This combination of posterior labral tearing, bone loss and humeral head impression fracture has resulted in recurrent instability. Based on imaging and clinical findings, Dr Cutbush explains why this patient requires an arthroscopic posterior bone block procedure combined with posterior labral repair to restore stability and prevent further dislocations.
