Botulinum toxin is used in many difficult to treat urinary problems. These could include patients who have refractory storage symptoms. That means they've got frequency urgency, which is not responsive to lifestyle changes or to medication. In some of those patients, use of botulinum toxin can really make a huge difference to their quality of life with about 70 % patients responding and almost one third of patients becoming completely dry. The disadvantage with botulinum toxin is that the effect wears off ultimately after about 6 to 12 months, an average of about 9 to 10 months. But for those 10 months, it can really make a life changing experience for the patient. And thereafter, re-injecting is a fairly simple task. We also use botulinum in neurogenic problems, nerve-related bladder problems where storage pressures could be high. And sometimes those pressures might put the kidneys at risk. In some of those patients, for instance, patients with spinal cord injury or spina bifida related neurogenic bladder, injection of botulinum toxin can render the bladder safe and can also relieve urinary incontinence problems in these patients. There are some of the smallest subgroups of patients, such as patients with problems related to their sphincter, where injecting botulinum to the sphincter can make a difference to passing difficulties, but those are not as successful and we need to re-inject much more often. As such, the injection is safe, easy to administer, can be done on a daycare basis. And typically patients experience their benefits in about a week after the injection."
