(Juvenile) Xanthogranuloma (JXG) is a benign proliferation of the inflammatory cells, the histiocyte. It is most common in infants/young people and can be present at birth, although they can occur at any age. They are dome-shaped pink to yellow nodules.
Histiocytes are the Mystique (XMEN) of inflammatory cells - they can take on many different forms, ranging from spindled to epithelioid to giant cell morphologies.
Microscopically:
1. Initially, there is a sheet of histiocytes (appear like light lavender) with scattered lymphocytes and eosinophils.
2. Over time, these histiocytes become lipidized and foamy with Touton giant cells.
3. The epidermis is often thin with elongated rete ridges.
4. Eventually, they regress with fibrosis.
