Keeping records of clinical services is a basic legal and ethical requirement in healthcare, including substance use treatment. Record-keeping creates an accurate, written history that details all the important aspects of clinical care. Record-keeping is important for numerous reasons: it creates an archive to remember what transpired; the clinician is able to compare records to track treatment progress, it provides important information for other (treating) healthcare professionals, and records protect the alcohol and drug counselor from charges of malpractice or negligence.
In this introductory lecture, the legal and ethical requirements for record-keeping in substance use treatment in California is reviewed, including the contents of the treatment record.
